Support Coordination

The role of a support coordinator is to connect participants with services providers, assist participants and their carers to navigate the NDIS system, and help to achieve the participant's independence goals.
We can be as involved as little or as much as you like. It's all about participants having choice and control. We can help in many ways including:
For example:
A participant's goal could be to increase their independence skills (such as learning how to cook). We can find five local companies that specialise in support cooking classes, either one-to-one or in groups if the goal is also to encourage the participant's social skills. A participant's plan may also be incorrect and/or under-funded by the NDIS. We can help to navigate the review process and coordinate the evidence needed by helping the service providers word their support letters using the correct NDIS terminology. We have a very high success rate. Stewardship RoleAs part of our Support Coordination role, we can also offer a 'stewardship service' where we will regularly visit the participant in their home, checking that their goals are being met, and also that they are living in a safe environment.
Multidisciplinary Team SupportsThis NDIS service enables a coordinated multidisciplinary approach to support participants. It needs prior approval from the NDIA.
Recovery CoachingWe can provide support and guidance by working collaboratively with participants, their families, carers, and providers to design and implement a recovery plan to allow participants to achieve goals and build their independence.
